Fanny Bernstein Living Well with Parkinson’s Education and Exercise Program at North York General Hospital

Description

Goal: to support care-partners & empower seniors with Parkinson’s to gain knowledge in disease management and to establish a “healthy brain” lifestyle to delay disease progression.

Services include:

(1) Individual consultation by:  specialized pharmacist and/or physiotherapist

(2) Seniors with early stage of Parkinson’s disease (within 5 years of diagnosis) may join the 8-week group education and exercise.

Topics covered include but are not limited to:

  • Overview of Parkinson’s disease and coping with the diagnosis
  • Recognition and treatment of motor and non-motor symptoms
  • Education on cognitive function, diet, and speech & swallowing
  • Mindfulness and relaxation training
  • Advanced care planning and community resources

Area of Expertise

The program is presented by the inter-professional team with many years of experience working with people with Parkinson’s. Team members include pharmacist, physiotherapist, occupational therapist, social worker, dietitian, recreational therapist, nurse practitioner and physician.
