People with Parkinson’s Disease (PwP) can access an organized, well-developed, multidisciplinary program within the community, to be well supported with their complex needs. The oversight of a highly experienced RN Care Manager facilitates timely problem-solving, collaboration with appropriate health professionals, and creates solutions to critical gaps in community care. Services are available for PwP in Ottawa, and virtual support for those in other regions of Ontario.

Area of Expertise

Nurse On Board Registered Nurse Care Managers are clinicians with 30 plus years of experience in various specialties, who in some cases are certified as Advanced Practice Neuro Nurses or otherwise have relevant clinical experience. We have developed 37 academically researched nursing care plans to address the unique needs of PwP, with support of the Registered Nurses Foundation of Ontario. We love working with PwP as we witness the benefit to quality of life through our customized nursing interventions; this is very rewarding for PwP, their providers and caregivers.
